DescriptionCollaborates and participates in the coordination of screening, assessment, and discharge planning activities in high risk patients and their families with complex psychosocial and economic needs. Utilizes expertise and problem solving skills to achieve optimal clinical outcomes within effective resources and timeframes. Promotes a seamless and safe discharge throughout the continuum of care. The position is based out of the Residency Clinic working closely with residents and faculty. This position will be working with, but not limited to OB patients, domestic violence patients, LGBTQ population, gender affirming care, and palliative care patients. Assists residents and willing to give feedback to residents about the clinical needs of social work.
Requirements: Minimum: LCSW required. Minimum of one full year experience and/or internship in a medical social work role in an acute hospital setting or agency. Must have prior experience with maternal-child populations in order to work in these areas of specialty; Preferred: Two ye Education: Preferred: Licensure: General: Must be licensed and maintain a current license or be licensing eligible.
